Do I need a serious illness to qualify?

Can I fill this out for a parent or spouse?

Yes — many people do. Enter the name and date of birth of the person on Medicare, and your own phone number and email if you're the best person to reach. We'll take it from there together.

Does this replace my doctor?

No — never. A licensed Pair Team clinician is involved in setting up your support, and your own doctor stays at the center of your care. Your advocate can't diagnose or prescribe. What your advocate does is make everything around your care easier.

What happens to my information?
